1) Social Security Card (of child)
2) Birth Certification (of child)
3) Proof of residency (utility bill/rental agreement/contract on home)
4) Physical/Immunizaion form Physical/Immunization Examination Schedule
5) Child must be 5 years old by September 30 in order to be eligible for Kindergarten.

Preschool applications: Please Note: completing the application does not guarantee a place in the preschool program. After receiving the applications, further screenings will be scheduled. Child must be 4 years old by September 30 to be eligible for Preschool.
